Out-of-Network Guide Disclaimer

The Levin Center is out-of-network with all insurance companies. The following guides are provided to assist you with calling your insurance company to check on your out-of-network benefits.

The Levin Center does not bill insurance. You will be responsible for paying The Levin Center directly, securing authorization for reimbursement from your insurance company, and filing claim forms.

Upon request, The Levin Center will give you a Superbill (receipt), showing your payment and procedure codes; however, there is no guarantee of coverage.

If authorization for out of network benefits is not obtained prior to your appointment with Dr. Bloom, your insurance company may deny your claim.

The Levin Center is not responsible for the information obtained using these guides.

Payment at The Levin Center

Clients have an option to request an itemized receipt of payment for services which will be fulfilled at the end of each month (therapy) or at the end of an evaluation (neuropsychological assessment). This may be used to submit to your insurance company. It is the client’s responsibility to handle this submission.
